Our storyWho we are, who we serve, and why it matters for Pennsylvania

Who we are
LTCMI is an Erie-based company that has created and operated shared housing and home- and community-based services (HCBS) in Pennsylvania for more than 20 years. Care is delivered through Affiliated LTSS (ALTSS), our operations management company.
Who we serve
Pennsylvania Medicaid members with the highest care needs — most come to us directly from a nursing facility. More than 95% of the people we serve need help with three to five activities of daily living, such as bathing, dressing, eating and moving around.
Our goal
Every Pennsylvanian who qualifies for nursing-home-level care should have a true home alternative: a house in a neighborhood, never more than three Consumers, with support around the clock. We are expanding county by county so that option exists in every part of the Commonwealth.
Why we're different
We serve nursing-home-level needs in a real home — helping people leave a nursing facility and return to the community, with fewer hospitalizations, falls and emergency visits, at lower cost to Medicaid.

What shared housing looks like

  • A real house in a residential neighborhood — not a facility, not a campus.
  • Never more than three Consumers per home — small enough for family to stay fully involved.
  • Part of the community — neighbors, home-cooked meals and activities tailored to each Consumer.
  • Care matched to each person — personal assistance averaging eight hours a day, with staff on site 24/7.
  • Built for the highest needs — people who would otherwise live in a nursing facility stay in a true home.
